LDL Cholesterol explained
The fraction that deposits into artery walls, and the number treatment aims at.
What the LDL Cholesterol test measures
Low-density lipoprotein carries cholesterol out to the tissues. More than the body needs deposits into artery walls — the process behind most heart attacks and strokes. There is no single cut-off that applies to everyone: what your LDL means depends on your overall cardiovascular risk, which is how the ACC/AHA guideline frames it.1
Reported as part of Lipid Profile.
What the result tells you
Why it is screened at all
A high LDL has no symptom and cannot be felt. It works silently over decades, and by the time it announces itself the damage is done.
Never read in isolation
The doctor weighs it against blood pressure, blood sugar, smoking, weight, age and family history before deciding anything.
High and young, with family history
Raises the question of familial hypercholesterolaemia — inherited, not a diet problem, treatable, and worth identifying because it runs in families.
When the number is unreliable
LDL is usually calculated from the other three figures. Very high triglycerides break that calculation, so a direct LDL is requested instead.
How to prepare
- Fasting
- 9–12 hours
- Best time
- Morning, with the rest of the lipid profile
- Avoid before
- Alcohol for 24 hours
- Worth knowing
- A calculated LDL depends on the triglyceride figure, so the fast matters more here than for cholesterol alone.
What can move the result
A result outside the usual range does not on its own mean something is wrong. These are the everyday explanations a doctor considers first.

Commonly: diet, weight and activity; familial high cholesterol, which is inherited; an underactive thyroid; very high triglycerides, which make a calculated LDL unreliable; cholesterol medication, which is designed to lower it. A single result is not a diagnosis. Values are read against the laboratory's own reference range and alongside your history, your symptoms and the rest of the panel — which is why every package includes a doctor to go through them with you.
