What the Testosterone test measures

The principal male sex hormone, made mainly in the testes. It governs libido, muscle and bone mass, red cell production, energy and mood, and falls gradually with age. The sample should be taken in the morning, and a low result confirmed on a second test before it means anything — the Endocrine Society guideline is explicit on both points.1

What the result tells you

Low, with symptoms

Explains a cluster easily blamed on stress or ageing: low libido, erectile difficulty, tiredness, loss of muscle, low mood and poor concentration.

Timing decides the number

Testosterone peaks in the morning, so a low afternoon result is repeated before conclusions are drawn.

Borderline with no symptoms

Usually just a borderline result. The number alone decides nothing.

A confirmed low is investigated, not just replaced

Weight, sleep apnoea, thyroid problems, some medicines and pituitary conditions all produce it, and several are reversible.

What can move the result

A result outside the usual range does not on its own mean something is wrong. These are the everyday explanations a doctor considers first.

Acute illness, which lowers it temporarily
