What it is
A simple extraction removes a tooth that is visible and can be loosened and lifted out. The area is numbed, the tooth is eased from its socket with gentle pressure, and the socket is checked and packed. You feel pushing, not pain. AED 229 for an incisor, AED 329 for a canine or premolar, AED 529 for a molar, X-ray included.
A surgical extraction is needed when the tooth is broken at the gum, has curved roots, or is impacted — typically a wisdom tooth. It involves a small cut in the gum and sometimes dividing the tooth to remove it in pieces. The X-ray shows which kind is needed (AED 50 on its own if the extraction is then not done). We assess wisdom teeth but do not remove them here; where surgical removal is required, the dentist tells you at the consultation and refers you, rather than starting and finding out.
Extraction is a last resort, not a first one. Where a tooth can be restored predictably, keeping it is almost always the better long-term outcome, because a gap has consequences — neighbouring teeth drift, the opposing tooth over-erupts, and the bone in the area slowly shrinks. Healing, and how to avoid dry socket
A blood clot forms in the socket and is the start of healing. Dry socket is what happens when that clot is lost or breaks down, leaving bone exposed: severe pain two to three days after the extraction, often with a bad taste. It follows 1-5% of routine extractions and is more common after difficult lower molars and in smokers.1
Rinsing with chlorhexidine reduces the risk. The Cochrane review found a chlorhexidine mouthwash used before and 24 hours after extraction reduced the odds of dry socket substantially (OR 0.38, moderate-certainty evidence).1 You go home with the rinse and instructions.
For the first 24 hours: no rinsing, spitting, smoking or drinking through a straw, all of which can dislodge the clot. Soft food, the other side of the mouth, and painkillers before the numbness wears off rather than after the pain starts.
Replacing the tooth
A gap at the back is sometimes left; a gap you can see rarely is. The options are a bridge, which uses the neighbouring teeth, or an implant, which does not. Both are discussed at the consultation so that the extraction is one step in a plan rather than the end of one.
