What it is
Inside every tooth is soft tissue called the pulp. When decay, a crack or a knock lets bacteria reach it, the pulp becomes inflamed and then infected, which is the throbbing, keeps-you-awake kind of toothache. Left alone, the infection spreads to the bone around the root tip and forms an abscess.
Root canal treatment takes the infected pulp out and seals the space. The tooth is numbed, a small opening is made in the top, the canals are cleaned, disinfected and shaped with fine instruments, then filled with a rubber-like material and sealed. The tooth stays in your mouth and keeps working.
It works for most teeth, and the numbers are worth knowing. A systematic review of 63 studies found first-time root canal treatment healed the bone around the root in 68% to 85% of teeth on strict X-ray criteria.1 Measured by whether the tooth is still there — which is what patients care about — pooled survival was 93% at four to five years and 87% at eight to ten.2
Does a root canal hurt?
The reputation comes from decades-old technique. The treatment is done under local anaesthetic, and for most people the pain that brought them in is far worse than the appointment that resolves it. A very inflamed tooth can be harder to numb, and the dentist will take the time to get it fully numb rather than press on.
Afterwards, expect the tooth to be tender to bite on for a few days. Over-the-counter painkillers are usually enough. Pain that gets worse rather than better, or swelling, should be reported the same day.
Why a crown usually follows
A root-treated back tooth is hollow and brittle. In a cohort of 203 root-treated teeth followed for up to eleven years, those left without a crown were lost at six times the rate of those crowned afterwards.3 That is why the dentist will talk about a crown on the same day — not as an upsell, but because the root canal buys you the tooth and the crown keeps it.
The crown is quoted separately — zirconia AED 599 or PFM AED 399 — so you see the whole path before deciding, rather than discovering the second half afterwards. A front tooth with plenty of structure left can sometimes be filled instead.
Re-treatment: when a root canal done elsewhere has failed
A root-filled tooth can become infected again if a canal was missed, the seal leaked, or the tooth was never crowned. Re-treatment means removing the old filling material, re-cleaning the canals and sealing them again. It is more involved than a first treatment and is assessed case by case with an X-ray; where the tooth is cracked or too little remains, the honest recommendation is extraction, and the dentist will say so.
Root canal or extraction?
Keeping your own tooth is almost always the better outcome — almost. The decision rests on how much sound tooth is left above the gum, whether the root is fractured, the state of the bone, and whether the tooth can be restored predictably afterwards. Extraction is cheaper on the day at AED 199 and more expensive over a decade once the gap has to be filled with a bridge or implant.
