What it is
A composite filling is bonded to the tooth rather than packed into it. The tooth is numbed if needed, the decay is removed, the cavity is cleaned and conditioned, and the resin is placed in layers, each set with a blue light, then shaped to your bite and polished. Because it bonds to enamel, less healthy tooth has to be removed than for the old silver fillings, and the result is invisible.
The honest comparison with amalgam. Silver amalgam fillings are no longer routinely placed here, and tooth-coloured fillings are what almost every patient wants. It is fair to say what the evidence shows: in a Cochrane review of two large trials in children, composite fillings in back teeth failed at almost twice the rate of amalgam and had more decay around their edges, on low-certainty evidence.1 In practice that means a composite filling in a big cavity on a molar is checked more carefully at each visit, and a very large one is better replaced by an onlay or crown.
When a filling is not the answer
If decay has reached the pulp, a filling will not stop the pain and root canal treatment is discussed instead. If so much tooth has gone that a filling would have nothing to hold on to, an onlay or crown is the stronger repair. A dentist who explains why the cheaper option is not appropriate is doing their job — and so is one who says a small area can be watched rather than filled.
After a filling
You can eat as soon as the numbness wears off — composite is fully set when you leave. Mild sensitivity to cold for a week or two is common in a deep filling and settles. A filling that feels high when you bite should be adjusted, not tolerated; it takes two minutes and prevents a sore tooth.
