What the HbA1c test measures

Glucose sticks to haemoglobin inside red blood cells and stays stuck for the life of the cell. Measuring how much has stuck gives an average over roughly three months, rather than the snapshot a single glucose provides. The thresholds a result is read against — for diabetes and for the range below it — are set by the American Diabetes Association's Standards of Care.1

What the result tells you

It finds prediabetes

The range where sugar is higher than it should be but has not crossed the line. Reversible for most people, and the main reason blood sugar is on a screening panel.

It cannot be gamed

Eating carefully for a day beforehand does not move an average. No fasting is needed, which also makes it more practical than a fasting glucose.

For anyone already diagnosed

The number that tracks control over time. Rechecked every three to six months — there is no point testing an average faster than it can move.

When it is unreliable

Anaemia, iron deficiency and anything shortening red cell life shift it. In pregnancy different tests are used instead.

How to prepare

Best time
Any time of day
Worth knowing
It does not matter what you have eaten — this is one of the few blood tests where that is genuinely true.

What can move the result

A result outside the usual range does not on its own mean something is wrong. These are the everyday explanations a doctor considers first.

Blood sugar over the previous three months, weighted to recent weeks
Anaemia and iron deficiency, which shift it either way
Conditions that shorten red cell life, which lower it misleadingly
Pregnancy, where different tests are used
Recent blood loss or a transfusion
