What is Dermal Fillers?

Dermal fillers are hyaluronic acid gels of varying thickness placed under the skin to replace lost volume or define a contour. Different products behave differently — a soft gel for lips is not the product used to support a cheek — and choosing correctly matters more than the brand name.

Hyaluronic acid fillers can be dissolved with hyaluronidase, which is a meaningful safety advantage over permanent alternatives.

It is worth knowing what actually goes wrong. A meta-analysis of 19 randomised trials found swelling, lumps and firmness were significantly more common at the nasolabial fold than at the midface, lips or perioral area, while pain, bruising and redness did not differ by site.1 Where you are treated changes the risks, which is why the consent conversation is site by site.

On longevity, a meta-analysis of 12 trials and 908 patients found no significant difference in effectiveness 24 months after injection between hyaluronic acid with lidocaine and hyaluronic acid alone.2 The lidocaine makes the injection considerably more comfortable — it does not make the result last longer.

How it works

After mapping the area, the filler is placed at a specific depth using a needle or a blunt cannula, depending on the site and the risk of bruising. The gel gives immediate structure and also attracts water, so the final result settles as swelling resolves. Work is usually staged across appointments rather than done all at once, because it is far easier to add than to take away.

Expected results

You will see a change on the day, though swelling means the honest assessment comes at two weeks. Longevity depends on the product and the area — roughly six to twelve months for lips and up to eighteen months for structural areas like cheeks and jawline. A meta-analysis of 12 randomised trials and 908 patients found no significant difference in effectiveness 24 months after injection between hyaluronic acid with lidocaine and hyaluronic acid alone — the lidocaine makes the injection considerably more comfortable, not the result better.2

Who it may suit

Considered where volume loss or a lack of definition is the actual issue. If the concern is skin quality or laxity rather than volume, filler is the wrong answer and a good consultation will redirect you. Not suitable during pregnancy or breastfeeding, or over an active infection in the area.

Side effects & aftercare

Individual responses vary — the team explains everything before treatment and is one message away after it.

  • Swelling for two to three days is normal, and lips swell more than anywhere else.
  • Bruising is possible and can last up to a week; arnica and cool compresses help.
  • Avoid strenuous exercise, alcohol, saunas and facial treatments for forty-eight hours.
  • Seek help the same day for severe or increasing pain, skin that turns pale or dusky, or any change in vision — these are rare but urgent.

Frequently asked questions

It should not, if the plan is conservative and staged. Overfilled results usually come from treating too much in one sitting. Building gradually across appointments is slower and gives a far better outcome.
